- Diagnosis with an illness that cannot be cured and with a life expectancy of six months or less;
- In agreement with their physician, has a desire to receive hospice care;
- Wants compassionate professional care in the comfort of their own home or home-like setting, including nursing homes and assisted living facilities.

- Inpatient Care: We will make available as needed short-term inpatient care for pain control and symptom management at hospice approved facilities.

- Respite Care: We will make available 5 days of respite services as needed, for caregiver relief, on an occasional basis at hospice approved facilities.

- Routine Home Care: We will make available all professional services to the patient, caregiver, and/or family in the comfort of their own home.
